Check the bait or quest item, restock your ammunition and keep any trophy or defeated body needed after the fight.

Pick up three leeches from the Forest Island ground.
Give them to the lady by the lake for Leech Bait.
Use it to summon Giant Piranha.
Return the named boss trophy—not generic meat—to receive Desert Island coordinates.

Clear the smaller piranhas first so the swarm cannot interrupt every shot. Bait the boss jump, sidestep, then use the recovery window for damage.
